This brief glossary of terms provides easy-to-understand definitions for common terms associated with your health insurance coverage.

Coinsurance or Copayment: Portions of the cost of care an insured person is required to pay, while the insurer usually, but not always, pays the majority of the costs. A coinsurance is a percent of the bill, while a copayment is a flat amount regardless of the total charge.

Coverage: The health care services included as benefits in your insurance program.

Explanation of Benefits (EOB): A statement received from your health insurer, which explains how plan benefits were applied to a specific claim and the amount paid to you or your physician. The explanation should also show how much of a claim has been paid so that you can inform a secondary payor about any outstanding charges owed.

Medicaid: A health insurance program jointly funded and administered by the federal and state governments that provides medical care for specified individuals with low income and resources.

Medicare: A federal program that provides health insurance for individuals 65 years of age and older and certain medically disabled individuals.

Medigap: Private insurance purchased by Medicare beneficiaries to supplement Medicare payments.

Out-of-pocket maximum: A cap placed on out-of-pocket costs, after which benefits increase to provide for full coverage for the rest of the benefit year; also known as an out-of-pocket maximum (OOP).

Physician-administered drugs: Drugs that are administered by, or under the supervision of, a physician.

Reimbursement: The amount of payment by insurers for health care services.

Neulasta® Indication and Important Product Safety Information

Neulasta® is indicated to decrease the incidence of infection, as manifested by febrile neutropenia, in patients with nonmyeloid malignancies receiving myelosuppressive anticancer drugs associated with a clinically significant incidence of febrile neutropenia.

Important Product Safety Information

Splenic rupture (including fatal cases), acute respiratory distress syndrome, and sickle cell crises have been reported. Allergic reactions, including anaphylaxis, have also been reported. The majority of these reactions occurred upon initial exposure. However, in rare cases, allergic reactions, including anaphylaxis, recurred within days after discontinuing anti-allergic treatment.
In a placebo-controlled trial, bone pain occurred at a higher incidence in Neulasta®-treated patients as compared to placebo-treated patients (31% vs 26%). The most common adverse events reported in either placebo- or active-controlled trials were consistent with the underlying cancer diagnosis and its treatment with chemotherapy, with the exception of bone pain.
