Overview of Donation Application Process

Amgen’s donation application process includes the following steps:

  • Choose donation type
  • Read donation application requirements
  • Complete donation application
  • Submit donation application online
  • Print donation application
  • Combine printed donation application with required supporting documentation (Based on donation type) to create a complete unstapled application packet
  • Fax the complete application packet to (888) 758-7776
